Athletic Space Request Form

    If this is a special sporting event, i.e. tournament, shootout, fundraiser you MUST also complete the Special Event Application.

  13. Insurance:*
    Insurance MUST be submitted 14 days prior to anticipated start date, if insurance is not submitted by this time the use of the athletic space WILL NOT be permitted. 
    All insurance must name with respect to (reservation holder) event being held (name the park or city property), on (DATE/DATES OF EVENT), AND The City of Cortez is named as additionally insured. IT MUST say the place the event is happening at, the name of the event and the date.

    Please submit your insurance via email at dspeer@cortezco.gov or bring to Cortez City Hall at 123 Roger Smith Ave. Cortez, CO 81321.
