Insurance:*

I acknowledge and understand that insurance is required 14 days prior to my reservation and failing to submit insurance to the appropriate authority will result in a cancellation of my reservation request and any fees will be forfeited.

Insurance MUST be submitted 14 days prior to anticipated start date, if insurance is not submitted by this time the use of the athletic space WILL NOT be permitted.
All insurance must name with respect to (reservation holder) event being held (name the park or city property), on (DATE/DATES OF EVENT), AND The City of Cortez is named as additionally insured. IT MUST say the place the event is happening at, the name of the event and the date.
